DOCUMENTARY REQUIREMENTS:

• Updated Statement of Account (SOA) or Certification of Outstanding Balance from financing company/ bank

• Original copy of encumbered or leased to or mortgaged OR/CR upon release (photocopy upon application)

• Latest picture of vehicle

• Stencil of motor number and chassis number (in blue form)

• Latest original Community Tax Certificate (Cedula)

• Latest original Barangay Clearance

• TIN ID or any proof of TIN

• Documented Proof of Income
