QUALIFICATIONS:

• Business existing for at least three (3) years

 

DOCUMENTARY REQUIREMENTS:

• Mayor's Permit and Barangay Permit to operate business.

• DTI Registration (if single proprietor) or SEC Registration, Articles of Inc. and By-Laws, Board Resolution to borrow with Secretary Certificate for authorization to borrow, General Information Sheet and authorized signatories of corporation (if corporation)

• 3 months latest and original checking account bank statements (2 months with original negotiated checks; 1 month latest print out)
