• Stencil of motor number and chassis number (in blue form)

• Inspection fee receipt (from LTO)

• Photocopy of LTFRB Franchise

• Latest original Community Tax Certificate (Cedula)

• Latest original Barangay Clearance

• TIN ID or any proof of TIN

• Documented Proof of Income (if any)

• Basic Documentary Requirements
